Drug Interactions
Your doctor or pharmacist may already be aware of any possible drug interactions and may be monitoring you for them. Do not start, stop, or change the dosage of any medicine before checking with them first. This drug should not be used with the following medications because very serious interactions may occur: barbiturates (e.g., phenobarbital, mephobarbital), carbamazepine, ergot alkaloids (e.g., ergotamine, dihydroergotamine), ranolazine, rifamycin antibiotics (e.g., rifabutin, rifampin), ritonavir, sirolimus, St John's wort. If you are currently using any of these medications listed above, tell your doctor or pharmacist before starting voriconazole. Many drugs besides voriconazole may affect the heart rhythm (QT prolongation), including amiodarone, dofetilide, pimozide, procainamide, quinidine, sotalol, macrolide antibiotics (such as erythromycin), sparfloxacin, among others. Therefore, before using voriconazole, report all medications you are currently using to your doctor or pharmacist. Tell your doctor of all prescription and nonprescription medication you may use, especially: birth control pills, cyclosporine, tacrolimus, methadone, phenytoin, sildenafil, cilostazol, tolterodine, certain chemotherapy drugs (dasatinib, lapatinib, sunitinib), "blood thinners" (e.g., warfarin), cholesterol-lowering drugs ("statins" such as atorvastatin, lovastatin, simvastatin), anti-anxiety drugs (benzodiazepines such as midazolam, triazolam), calcium channel blockers (e.g., verapamil, dihydropyridines such as isradipine, felodipine), oral diabetic drugs (sulfonylureas such as glipizide, glyburide, tolbutamide), anti-cancer drugs (e.g., vinblastine, vincristine), proton pump inhibitors (e.g., lansoprazole, omeprazole, pantoprazole), HIV protease inhibitor drugs (e.g., amprenavir, darunavir, nelfinavir, saquinavir), HIV non-nucleoside reverse transcriptase inhibitors (NNRTIs such as delavirdine, efavirenz, nevirapine). Keep a list of all your medications with you, and share the list with your doctor and pharmacist.
