Post-Accutane and acne relapse

I am a 21 year old male and since i was 18 i have had moderate acne and have been on several antibiotics ranging from tetracycline to minocycline. I started isotretinoin(40 mg amnesteem twice a day) in June of 2006 and completed my course in November of 2006. The treatment was incredibly successful but as of early February i have been experiencing persistent breakouts. The breakouts are nothing severe but i constantly worry about it getting progressively worse. Being that my oil glands are still only around 50% of their original size i worry that my acne will only increase in its severity as my glands increase in size and produce more oil. I have visited my derm twice since January. He prescribed me Tazarotene in January and benzoyl peroxide in March that i have yet yo use. I am very persistent with my regimen washing with a mild neutrogena cleanser morning and night and am religious about applying my topicals.
1. How do i properly incorporate the benzoyl peroxide and the tazarotene into my regimen?
2. I have heard that topicals do respond better after accutane treatment. Do i have a good chance of having any luck with just topicals being that my acne has already relapsed?
3. I have thought about taking another course of accutane. My doctor has told me if i choose to do so i will take the same dosage as last time. I fear one side effect in particular and that is acne fulminans. Could you give me any knowledge that you know about this and also give me your opinion on me going on a second course of accutane?

It is time for you to add a gentle exfoliant such as a beta hydroxy lotion at night after you put on yout meds.
Beta hydroxy is able to penetrate the pore and encourage the removal of dead skin cells. this keeps them from staying around to mix with oil and bacteria.
You can get beta hydroxy at the drugstore.Give it 4 to 6 weeks to see and improvement.Then you might wnat to reconsider a second round of accutane.

what brand of topical beta hydroxy can be found at the store?  Also, if i am using other topical medications before i go to sleep, what would be a good way of using beta hydroxy?  Should i use it all over my face before i put on other topical medication or wait for normal medication to dry then put on the beta hydroxy? please explain ty.

Depending on the topical meds you aare using it is reccomended to use them during the day and use the beta hydroxy at night.Beta hydroxy has the ability to penetreate the pore and exfoliate the dead skin cell that pile up and contribute to breakouts.
I use paula's choice that I get from the cosmeticscop.com  at the drugstore I think it is nuetrogena but there are several on the market .Theey are located with the alpha hydroxy products.The staff should be able to direct you to selection.Paulas though is the best I have ever used.
Good luck and let me know how it works for you.I know after 4 weeks of using the beta hydroxy the difference was fantastic.
Dont forget to wear sunscreen
