Elevated liver enzymes

Hi, thanks for taking my question.  In Aug 2005 was found to have spiculated mass 2.5 cm that resulted in DX of infiltrating

ductal breast cancer.  Also had DCIS.  Clean margins were never

achieved, lymph nodes negative however tumor had invaded the vascular spaces.  Treated with 4 dose dense treatments of Cytoxan and Adriamycin; 36 radiation treatments.  Have had a terrible problem with fatigue.  Am on Femara and Provigil (for fatigue).  Last week on routine oncology follow up my liver enzymes were elevated.  Alk. Phos 122 (slightly) SGOT 151  SGPT 165.  Never elevated before.  I had been having symtoms of just not hungry and feeling very full when I did eat.  Had CT of liver, pelvis and chest.  CT of liver showed cyst (they say hemangioma) middle lobe and fatty liver (ugh!!!)  They want to repeat liver enzymes and if still elevated mentioned liver biopsy.  My question is why?  Can "fatty liver deposits" hide or mimic anything serious.  Other than just severe fatigue and

loss of appetite I feel fine.  On the day I finished radiation, I just about danced out of there.  I though "NOW, NOW I can move forward without having the word cancer in my face every morning - seems things keep popping up and it just hasn't happened.  I have a postop seroma that we have just decided to leave alone - radiation may have sealed the edges and it is something I will have for a very long time or maybe permanently.

Is elevated SGOT and SGPT indicators of breast metastasis to liver?   Again, why suggest a liver biopsy if all is ok.

Dear Naniam:  Liver enzymes can be elevated for many reasons.  These vary from transient elevations due to certain medications to hepatitis, cirrhosis, and even cancer.  In general, elevated enzymes indicate some tissue damage.  A liver biopsy may be recommended if the enzymes remain elevated in order to determine what might be causing the elevation.
