emphysema-end stages and dime size density on lungs-what can that be.

My Dad is 85 has end stages of emphysema bad arteries bad heart and now they found a dime size density on his lungs. What can that be? What signs do I look for - for his end. Please I need answers so I can cope better with myself and help my Dad.
