help for congenital hypothyroidism

Dear Docs:



I have questions regarding my son’s congenital hypothyroidism (CH).



We were informed at 21st day that the new born screening exam found my son may have the CH. The screening result was: T4=10.5ug/dl and TSH=36.5uU/ml.



The serum inspection on the 22nd day confirmed the CH. The serum results were: T4=3.7ug/dl, free T4=0.5ng/dl and TSH=189uU/ml. My son was treated by L-thyroxin from 28th day with the initial dose of 25mg for first three days and 37.5mg after that. There was no radioactive thyroid scan at that moment.



After 14days medication, we brought my son to see the pediatric endocrinologist. My son was checked of thyroid by ultrasound, T4/TSH serum level and X-ray bone age. Now we are quite confused by the results: first, the ultrasound inspection by thyroid shows the correct position and size of thyroid. Second, the T4=12.5ug/dl and TSH=0.3uU/ml !!!???



We understand the ultrasound inspection may not be very useful to diagnose the reason for CH. However, my wife and I don’t understand why the TSH drops so greatly from 189 to 0.3 which even lower than the normal range (0.4~9.1). We know the T4 level may be normal after two weeks and TSH level should take 4~6 weeks to get normal.



Our pediatric endocrinologist could not answer the questions.



Now, our questions are:

1. Is that normal for such a great dropping of TSH during the congenital hypothyroidism treatment within the first two weeks?

2. If the TSH is below the normal range, doesn’t that mean my son has the hyperthyroidism?

I am not trying to sound like I am turning you away, but my strength as far as giving advice lies with type 1 diabetes.
