Congenital 4th nerve palsy (moderate misalignment, double vision, no "lazy eye" developed). Developed downbeat nystagmus with other neuro symptoms in 2003. Diagnosed with Arnold Chiari Malformation. Decompression surgery successful, nystagmus remains. Successful strabismis surgery to bring images side by side and level but not fused. Recently developed visual snow (slight "static" pattern at all times-upon waking, in dark, in light, no change in intensity or pattern), loss of detail at distance (esp. facial expressions near, recognition of person at 15 feet or more), horizontal nystagmus-type movement when eyes are shifted mere degrees off center, resulting in elliptical pulses. Straight ahead is pure downbeat. Tested by opthalmologist and neurosurgeon. MRI and optical tests showed no abnormality, esp peripheral field test and retinal exam. Snellen test shows correction 20/20 and 20/25, tested separately. No "good reason" for symptoms. Nothing is structually wrong with my eyes or brain, my prescription is ideal, yet everything seems distorted. Patterns (esp stripes and squares) seem to "ripple" and move. Concentrating on patterns or pictures for any length of time causes severe headaches. Visual clutter makes it difficult to find things. Any ideas, suggestions, or possible diagnosis?

BTW-I'm a 33yo female. The ACM was triggered by an epidural used during childbirth. Let me know if you need any more information.

Oops...I forgot to mention...the opthalmologist said I had developed a sixth nerve weakness when I saw him last-after the latest set of symptoms. He sent me to the neurosurgeon, who found nothing wrong. Sigh.

Any possibility that it might be atypical migraine? If so, then appropriate migraine treatment might break the cycle.
