This forum is limited to prevention of HIV and to safe sex in general. If you believe you might have been exposed to HIV and want help to judge your risk, would like advice about HIV testing, or have questions about the effectiveness of condoms or the risks associated with specific sexual practices, this is the site for you.

IMPORTANT

No questions will be accepted on the treatment of HIV/AIDS or its complications, viral load, and similar topics. If you have questions about a specific STD other than HIV/AIDS, please visit the STD Forum. Questions that do not pertain to the above topics will be removed from the forum.

If you have not done so, please review other threads in our archives for questions similar to yours and Dr. Handsfield's replies. Questions that duplicate other frequent ones, for which abundant replies exist, and that have little educational value for other forum users, will be DELETED WITHOUT RESPONSE. YOUR PAYMENT WILL NOT BE REFUNDED. The most common examples of such questions are those about low risk exposures to HIV, such as oral sex, condom- protected intercourse hand-to-genital exposure, and nonsexual contact with possibly infected blood or body fluids as well as symptoms of early HIV infection.

hi, i had sex with two guys yestereday ,i m gay, while doing coke and popers.  I sucked both men,one of them i tasted their presum.  the both rimmed me for a very long time. I guy was going to **** me but complained the rubber made is **** get small.  I appologized and stated i dont bareback.  the guy was cool with that.  a few minutes later with out me realizing he started cumming on my back.  i dont know if any of his semen got on my ***.  It was not near it like humping it or anything but he was standing over it.  we contimued to play and i was continually rimmed.  he kept hinting how he wanted to **** me but the rubber problem so now i think to my self he really wants to bareback me, this means his chances of being positive have gone up exponentially. nothing else happens, but i am freaked out now to the point i had a freind start me on post exposure prophylaxis.  he did this to calm me down.  I need a third party to tell it to me straight that my chances are nill and zero and should immeditaly stop taking the pep or tell me may was well keep taking them, or what ever advise i can get.

I'm happy to be the reassuring third party.  First and most important, congratulations for sticking so firmly to your dedication to safe sex, especially in the setting (coke, poppers, etc)! You saved your self from a real risk of catching HIV; and if you stick with the program, you can be confident you'll never get HIV.  My only additional advice along those lines is that it would have been good to insist on knowing your partners' HIV status before you got started, so that you wouldn't need to be so worried if the other defences (e.g. condom intent) fail.

The risk of HIV from oral sex or from skin contact with semen or other infected secretions is zero.  No data are available on rimming with respect to HIV transmission, but almost certainly the risk is extremely low, probably on the same order as the risk by oral sex.  As for semen on skin coming in superficial contact with your anus, that also is too low risk to worry much about--certainly miniscule compared with having a guy come in your butt.

I'm disturbed by your getting started on PEP by "a friend".  Non-medically supervised PEP is really, really dumb.  I cannot think of a better way to foster the spread of drug resistant HIV strains, especially if people without medical supervision don't stick with the program and stop treatment early.  It doesn't much matter in your case, since you couldn't have been infected, and you should stop the drugs; or immediately see an HIV-knowledgeable health care provider and follow his or her advice.  But please don't do it again.  If you are bottom with a guy without a condom and especially if he is known to have HIV, PEP would be warranted--but not in this situation.

I trust you're getting HIV tested regularly.  When you can make yourself do it, also ask and share your own HIV status.  And consider whether you want to place yourself in such high-risk, high-temptation settings.  But if you do, but stick with your current practices, you'll do fine.  Pass the word to your buddies.

Good luck--  HHH, MD

Doctor -- I can't believe you told this gentleman that he has ZERO RISK! Granted he has VERY LOW RISK, but it is not ZERO considering he PERFORMED ORAL and tasted semen.  Don't you think that was a little too confident of a response?

OK, "zero risk for practical purposes" (for any single episode of performing oral sex).  It remains true that the vast majority of persons with the exposures billyboy describes, even if repeated day in and day out, will never get HIV.  But there is a small risk of HIV by performing fellatio.  None documented by receiving oral sex.

Steve, I mistakenly hit a button on the moderator's response page that banned you from further posts.  Your privileges will be restored, but I need to ask the web manager to do it.  It might take a day or so.  Sorry!

Hi Doctor,

Steve is my brother.  I'm Rick.  Two different screen names.  Thank you for giving him great advice the other day.  It seemed to straighten him out.  Many thanks.

Just out of curiousity - does precum have a lot of the virus in it?  I know oral is extremely low risk, but since most people performing oral on a man would be exposed to just precum is that enough for transmission to take place?

Good question, Dr. HHH?

I'm not aware that there has ever been any research on HIV in pre-ejaculate fluid.
