Fallopian Tube Cancer Recurrence??

My mother had both Fallopian Tube and a seperate endometrial papilary serous cancer 2yrs ago. (I know, what are the chances) She had a historectomy and 4 rounds of carboplatinum and taxol/taxotere (she switched to taxotere as a result of extreme neropathy). It has been just about 2yrs. She was without incident until June when she had right sided pain and a CT was ordered. Retropernial paraaortic lymph node was enlarged at 1.7cm. A PET scan was done a month later which showed metabolic activity in the lymph node and pelvic area, generally confirming the CAT. I think the Drs have been entirely too slow about length of time between tests, but at any rate a second CT and biopsy were done last Thursday/Friday. The pathology has yet to come back, but the lymph node is now 3.8cm. Granted while there are always other possiblities, I am assuming cancer... If so what are the next steps. In my opinion surgery would be the next step and surgery very soon. Thats pretty aggressive and at 3.8cm abdominally it seems far too large to use radiation or leave in the body. After that... Chemo? And if so, what Chemo? Certainly Platinum as that is the best agent for this type of cancer, but the taxins just do not seem to work for her given neropathy... What would be a good choice for another agent? Would radiation be indicated? I'll update this after pathology and "plan," but what would you make of this situation? I must say that I am very displeased that after a CT that was noteworthy it took 1 month to get a PET, which generally confirmed the CT findings, and then another 2 months before biopsy

Hi Mark,

please see my recent discussion about secondary cytoreductive surgery posted on 9/23/2006). That discussion would apply to your mother as well.. If your mother has no other signs of recurrence, I would remove the lymph node. Then she could consider another round of chemo such as carboplatin or carboplatin with gemcitabine, or others.



Yes definitely another option is focused radiation to the area of the lymph node. These will be good points to bring up with her oncologist to see which option fits her the best.
