Ovarian cytst due to ORS

In April, I had my last ovary removed due to a 9cm ovarian cyst.  Since then, I have been having lower abdomen pain and lower back pain.  After undergoing many test to rule out other issues, I finally had a tvus done last Friday.  To the tech's surprise, I have a 2.5 cm cyst but no ovaries.  I have been discussing the posibility of having Ovarian Remnant Syndrome with my doctor.  Now I am going to see a gyn/onc on Wednesday.  My question is, if the cyst is fluid filled, then there is no way it can be cancer right?  Do you run a greater risk of getting cancer with ORS?  Thank you for any advice that you can give me.

Hello,

The 2.5 cm cyst seen on transvaginal ultrasound (TVUS) could be a retained ovary with a cyst in it. It could also be a small fluid collection between some adhesions. One possible test you could get to sort this out is a blood test to check for ovarian hormone secretion (that would be estrogen, FSH (follicle stimulating hormone) and LH  (luteinizing hormone)). Of course if you are through the menopause, these hormone tests would be low even with a cyst.



It is not possible to make a diagnosis of malignancy or non-malignancy based on an ultrasound. There are certain ultrasound properties that are associated with a malignant cyst on the ovary. These properties include: size greater than 5 cm, complexity of the cyst, and solid elements within the cyst.



I do not think a retained ovary in an of itself causes a predisposition to malignancy. For women who have a strong family history of ovarian cancer, or who have had ovarian cancer themselves, there is a risk of malignant change in an ovarian remnant. Otherwise, there is no increased risk over the general population.

Thanks for your response to my question.  I am not having any menopausal symptoms (I am only 31).  What should be done with such a small cyst on or within the ovary remnant?  Thanks again for you help.  Kasie

Hi , then I would definately recommend getting an estrogen, FSH (follicle stimulating hormone) and LH ( luteinizing hormone) blood levels done. That will tell you abolutely if this is an ovarian remnant or not. Then if it is not bothering you, leave it alone. Maybe check a follow up ultrasound once or twice a year.

About 10 weeks ago I started getting pains in my lower right abdomen. I went to my gyno who told me I had a cyst that ruptured and alot of fluid left that would go away eventually. She told me to come back right after I get my next period because the lining of my uterus was a little thicker than it should be. I went back and the lining of my uterus was normal but the ultrasound tech said I had a lot of cysts on my right ovary and a few on my left. When I went back for a consultation with my gyno she put me on birth control and said to wait 3 to 5 months and if the pain was unbearable we would discuss surgery. It has been 6 weeks and the pain seems to just get worse and move all around. I have pain on my right side (upper and lower abdomen) and horrible pain in my back and right leg. I am worried that the cysts might be getting bigger, or there is something else wrong like appendix, gallbladder,etc. Any incite would be greatly appreciated.

Hi There, it sounds like you should talk to your doctor about a laparoscopy where the doctor will take a look inside  through a small incision.
