surgery during chemo

I am asking about surgery during chemo which will delay chemo so there could be some risk with cancer. My sister's CA-125 started at 195 and after second chemo is 17.2. Had 3rd chemo and she is getting a CT scan 3 days before 4th chemo. She has a lot of burning pain in vaginal area after washing due to recto-vaginal fistula. Would it be wise to due surgery before completing all chemos or would that be too risky for cancer to grow or become more resistant? If the CT is clear and she gets a PET and that is clear would the surgery then be on option? It would be a temporary colostomy to let fistula heal but it would delay the remaining chemos.
