follow up ultrasound and ca125 results and question

Hello Dr. Goodman. I have emailed previously and now have the results of my follow up ultrasound after laparoscopy in april. My findings have been benign. My gyn/onc told me that my ultrasound results were very stable and that the chance of me having ovarian cancer was very small. Then 2 days later the office called and told me my ca125 was 110. As I have told you before the pre lap ca125s were 64 and 75. Post lap 164 then 89. Now 110. I am held prisoner by these tumor markers. The last one was drawn on day 6 of my menstural cycle. I am told to have it redrawn on day 9 of my next cycle. You have stated before that these markers are watched to make sure they are not doubling and tripling. Since my initial one was 64 and now 110, that is almost doubled. Is it possible than stage 4 endo and my large fibriods is causing a constant fluctuation in this? At what point does a DR. choose to do a second lap or just keep watching the numbers? I finally felt like I was going to be ok and now I am back at worrying again. Please advise.

Hi There,
That is a tough call. The CA 125 is measuring what is going on with your endometriosis and fibroids. If you are symptomatic from these conditions with pain, trouble with bowel or bladder, abnormal bleeding and you have completed your plans for having children, you could consider a complete removal of the uterus, ovaries and fallopian tubes.

If you wish to preserve fertility and/ or are not symptomatic - I would suggest a repeat CA 125 in a month.

You should probably go over the findings of your surgery with your doctor to be sure you understand what was seen and what could be be seen -for instance if there were adhesions and it was not possible to see one of the ovaries)
