Beta HCG level How accurate are they during prgnancy?

Hello,
  I beloved that I was impregnated on March 11,2007 I planned it after that I went to the emergency room twice once for vagina bleeding and discharge that was on the 25th of march when I was there they gave me a pregnancy test that according to them was negative.. I went again for on march 28th in which they gave me a blood test and determined that I was pregnant my BHCG level was 500 by then I was only 17 day pregnant that 2 weeks and 3 days was my BHAG level at the right level? I had me first prenatal visit on april 13th in which according to the doctor I was a little over 4 weeks... But now that I am 18 weeks my uterus is measuring 24cm what does this mean? I am confused I know there is no way that I could me 24 weeks pregnant, is there any way that my uterus can be swollen, or there is alot of fluid in my uterus or, just that I am having a bigger baby than usual. and for my last question If I was 9 week pregnant how would a doctor determine how far along i was seeing that the baby is still kind of small? would that do a uterus test, a BHCG test, or can they determine the size by look at an ultrasound.

Hello,

It is not uncommon for pregnancy dating by ultrasound to differ from dating determined by the last menstrual period.  Ultrasound is the most accurate way to determine gestational age when performed early in pregnancy (<12 weeks).  It loses its accuracy as a pregnancy progresses in to the second and third trimester.

If your dating (whether by ultrasound or last menstrual period) places you at 18 weeks and you are measuring 24 weeks on exam, your physician should review your dating to make sure it is correct, and evaluate you for what is considered size greater than dates by doing the things you noted: checking an ultrasound to look at the fluid around the baby, calculating the fetuses estimated weight, and looking for anomalies.
