High PSA>20, Normal Ultrasound......Next step?

Dear Doctor,

I am from India. My father, 63 yo, in his usual health had a severe back pain 5 weeks ago. During his visit to the urologist, he all told to do renal function test, CBC, electrolytes, PSA. That time his PSA was 28. DRE was normal and rectal U/S also normal with no mass. Doctor advised him to treat with antibiotic (cefuroxime) for 1 week and told him to come after 3-4 weeks later. Last week when he went to do repeat PSA it was 26.8. He has some back pain after a 2 week course of non-steriod pain killer.

We are pretty worried about the situation. Can you suggest me please what will be the best course of action. Should we do more blood test like Acid phosphatase, free PSA, PSA density or go for prostate biopsy.

I will be highly oblige if you let me know.

As the doctor stated, the biopsy seems to be an appropriate next step.  You'll get more definitive information.

It is a simple procedure and many doctors have the ability to do it in their own office.

They will likely give your father a mild sedative before the procedure and you'll probably need to drive him home from the office.  He’ll probably need to take it easy for the rest of the day.

In my experience, they’ll have the results within one week.
