I didn't think your initial symptoms were anxiety-related, but your persistent/recurrent symptoms may be. The urologist's report that of a negative urine test doesn't mean much; a standard (midstream) urinalysis often is normal in NGU, and regular tests for chlamydia and other bacteria usually are negative. The amount of antibiotic you took is irrelevant; it depends on which antibiotic(s) were used.
If your symptoms persist, you should see an STD expert. Your urologist may qualify, but most do not. Consider your local health department STD clinic. Go early in the day, immediately after arising, and do not urinate until you are examined. That is the best way to assure an accurate assessment of possible urethritis.
