Can this be a STD?

I'm a male that had a felatio (unprotected) performed on me by a female for about a minutes.  I didn't know here sexual history.  After my symptoms I found out that she get cold sores but, at the time of the felatio cold sore where not present.  About a week or two later I started to have a burning/stinging sensation in my penis and in my urethra. It burned when I urinated and there were no blister or lesions. The meatus was itchy and senestive underneath.

I went to the doctor and got tested for UTI, all STDs except for herpes and HIV. He also gave me antibotic for the infection. The test came back negative. About a month later the pain returned. I went to see a Urologist and and he gave a Herpesselect I and II test and a HIV test. Everything came back negative except for HSV-1 test.   I never had a oral cold sore or blister.



The irritation went away after two weeks. The symptoms came back for 5 weeks and currently I'm having vague itching at the tip of my penis and a slight sting after I urinate. The first episode was the most painful one.   Can this be herpes, hiv, or some other STD? Can  UTIs recurr in this fashion and if it does how long does it persist?  Please help me.

It is possible you acquired nongonococcal urethritis (NGU), likely due to your partner's normal oral bacteria.  Did the first doctor look for evidence of urethritis, e.g. by looking at urine or a urethral swab for white blood cells?  In any case, you have tested negative for all the important infections.  Your positive HSV-1 blood test is more likely to be the result of a childhood oral HSV-1 infection.  (Most HSV-1 infections are asymptomatic; absence of cold sores doesn't mean anything.)  If your symptoms keep recurring, you could have a culture test for HSV from your urethra, but this is a very unlikely possibility.  Bacterial UTI is rare in men and not likely in your case.



Nonchlamydial NGU often recurs after treatment.  If one of your doctor's is able to confirm urethritis (visible discharge, or by urine or swab test as implied above), then a repeat course of antibiotics might be warranted.  But the main thing is not to worry very much.  Nonchlamydial NGU has never been shown to cause serious long term health problems in men or their female partners; and even without treatment, your symptoms are likely to fade.



Finally, your symptoms might be at least partly related to anxiety, magnifying otherwise normal body sensations.  But you can judge this possibility better than I can.

Thank you so much for your help.  I don't think the inital epidsode was due to anxiety. But I think it is a possibility that the symptoms came back due to anxiety.  It is realy frustrating because the symptoms will not go away.  I 've took enough antiboitics to cure a horse.  He test me for evidence of urethritis but he didn't tell me that the results so I'm assuming that the result were negative.  Does this sound like genital herpes(HSV-1)?

I called mu Urologist and the urine test came back negative.  What could these symptoms possibly be?

If your positive blood test for HSV-1 is due to childhood infection, which is likely, you are immune from catching HSV-1 again.  It is conceivable the blood test and your symptoms are due to HSV-1 acquired in the encounter your describe, but extremely unlikely.



I didn't think your initial symptoms were anxiety-related, but your persistent/recurrent symptoms may be.  The urologist's report that of a negative urine test doesn't mean much; a standard (midstream) urinalysis often is normal in NGU, and regular tests for chlamydia and other bacteria usually are negative.  The amount of antibiotic you took is irrelevant; it depends on which antibiotic(s) were used.



If your symptoms persist, you should see an STD expert.  Your urologist may qualify, but most do not.  Consider your local health department STD clinic.  Go early in the day, immediately after arising, and do not urinate until you are examined.  That is the best way to assure an accurate assessment of possible urethritis.

Thanks again for your help.  I'm sorry for asking so many questions.  I've been reading that HSV symptoms varies.  I've been to several doctors and they cannot give me answer.  Is it fair to say that I aquired HSV-1 from my encounter or is the possibilies is slim enough to forget about it and move on with my life?  My psychologist insist that it is due to anxiety.  Do you think it is worth going to a STD clinic or just wait it out until the symptoms fade?
