BV and HIV Transmission

Hi Dr. ~  Thanks for taking the time to review my question.  I have had two encounters of unprotected cunnilingus / fingering (don't know the technical term for that!) with two different partners over the last 3 months.  They are both lesbians if that matters.  I just went for my pap and it came back normal, but with signs of BV to which they are prescribing me something to treat it.  I also had a full STD panel, including HIV b/c I was concerned about possible exposure (just anxious I guess) and have not yet received those results back.  Does the fact that I have BV increase the liklihood of transmission via unprotected cunnilingus / fingering??  Thank you in advance.

First, HIV transmission by cunnilingus or hand-to-genital contact has never been documented--which is why HIV transmission between lesbian women is virtually nonexistant, if it occurs at all.  When lesbians get HIV, it usually is from injection drug use or sex with men.  Second, lesbians are at risk for bacterial vaginosis; in fact, the risk of BV may be even greater among women who have sex with other women than in strictly heterosexual women.  (Some of the main research in this area has been done by my close friend and colleage, Dr. Jeanne Marrazzo of the University of Washington--in case you want to look up any of that work.)  Third, BV indeed increases the risk of HIV transmission, other things being equal.



However, since the risk of sexual HIV transmission among women is microscopic, even an enhanced risk in the presence of BV is meaningless.  This is an issue primarily for heterosexual women whose male partners are at risk for HIV.  I wouldn't lose any sleep over it, and would not have recommended HIV testing had you asked about it before having the test.  But no harm done; assuming you have no other risks for HIV, you can expect a negative result.

Dr. ~  Thank you so much for your response, and yes, you assume correctly that I do not have any other risks exposures.  I like clean livin!  haha  I was just thrown off by the BV...and OF COURSE, the pap results have come in before my HIV result, even though they were all taken in the same day.  Murphy's Law I suppose.  I will certainly look forward to receiving a negative result, and will try not to lose sleep over this.  Thank you for your work.

Well, Doc...you did it again.  :)   I received my negative result today...for HIV, syphillis, and chlamydia.  Lurkers / Worried People...listen to what this Doctor says, use condoms always, no exceptions, and be healthy!

One last question.  Why do they call it a 'culture'?  I always thought a 'culture' was when you took a strep throat culture, or a pap smear, or something like that.  But, when I called my DR for results, they said 'your cultures were negative' and I had to ask 'what cultures'???  Then they explained to me that my pap was fine and my 'cultures' were negative, I guess there is some privacy issue with them being totally specific over the phone, but I got the picture that my tests were negative, as suspected (hiv, syphillis, and chlamydia)  If you have time to answer, great...if not, I'm sure I'll get some info on the net.  I just didn't understand why that terminology would be used instead of just saying, 'hey, your tests were negative'...

Blood culture.  A culture can be defined as any specimen collected for testing or evaluation, (ie. Blood, Fluid, skin cells, skin, ect).

ryn21 probably is right--just common jargon.  Technically sloppy, and many providers take care to distinguish between (say) a chlamydia culture vs a chlamydia DNA test; but just a habit of speaking.
