Oral sex with Canker Sores, Buring Urethra, HIV

I have read many of your answers about STD’s and oral sex, however I would like to know what your thoughts are on my situation.

3 weeks ago I met a girl and had a one-night stand. She gave me unprotected oral and I gave her the same followed by protected sex. The next morning I woke up and noticed that I had 2 large canker sores in my mouth, which I get pretty often, but I did not notice them before performing oral, which of course have turned me into a worry wart!

Two weeks went by and I started to feel a dull burning sensation in my Urethra, which has continued for about a week. It does not burn at all when I urinate and there is no discharge. I am living out of the country right now so getting tests done are not really an option so I self treated myself with the following.

1gram of Zithro all at once just incase it was chlamydia and then 5 days of Cipro just in case it was something else.

I still am feeling the slight burning after 3 days of Cipro it has not gotten any better or any worse. Can you recommend any other treatment I can buy any medication over the counter here and would much rather take your advise then one of these so called pharmacist that probably don’t have any training.

My biggest concern is possibly catching HIV from giving oral to this girl with Canker sores in my mouth, what do you feel the risk of this is.. Any advice would be greatly appreciated!

Thanks and take care...

HIV was not a risk, even if your partner was infected (which she probably was not).  Even with soon-to-appear canker sores, you have no worries from cunnilingus; and HIV is rarely if ever acquired by penile exposure to fellatio.  Your symptoms sound more like anxiety than an infection, but at this point you'll never know for sure.  The cipro and azithromycin would have cured any NGU or other infection, and also prevent any future diagnostic testing.  Do not attempt to treat yourself further; if your symptoms persis, see a health care provider.

Good luck--  HHH, MD

Cunnilingus is considered "less risky", that's understood, but what if an individual was recently infected with HIV (maybe one, two, or three weeks prior to an exposure). How would this affect the odds of transmission.

As far as I know, it wouldn't really affect it. Think of it this way: there have been only a couple of documented cases of HIV transmission through cunnilingus, even though there are literally millions of people having oral. I think it's clear that transmission this way is very, very rare - fresh infection or no. :-)
