Chronic Urethritis and prostatitis and worried about HSV

I am a 21 yr old male who had sex a year ago with 19 yr old. I was previously a virgin. I had unprotected oral and vaginal sex with condom for 5min total. Condom came off briefly while in her. After this i performed oral on her for 2min. Next morning my prostate area was soar/ painfull, next day penis tip had pinching and i experienced urethritis, this got worse becoming chronic with bouts of hard pain. I examined myself daily for sores convinced from it was HSV-2. I went to the doctors a month later and tested for chlyamidia and ghonorrea, both neg. 1 1/2 months after sex had 1st coldsore ever. Went to the docs 6 months after sex and he said the pain was inflimation caused by large Vericocelle vain in scrotum Ive had for 2yrs. He tested my urine for "bacteria" was neg. Went to the urologist 8 months after sex and he said it was the vericocelle as well. Its been just over a year, did notice couple painless red bumps on the bottom of my penis shaft once. Now, after a year i notice a small circular red area that is inside the tip of my urethra when i open it up. Is this proof of herpes and the first sore i have noticed since its in my urethra. It doesnt hurt when i touch it, no fluid discharge and doesn't hurt when urinating. Many people online report having chronic urethritis and herpetic neuralgia years on end with no sores and test positive for hsv-2! Could my symptoms be caused by herpetc urethritis, vericocelle? or neither?

Herpes doesn't cause this sort of problem or any of the specific symptoms you describe.  You can be sure that if your provider requests a test for antibody to HSV, your HSV-2 result will be negative.  (Or, in the off chance it is positive, I still would conclude that herpes does not explain your problems.)  (If your oral lesion indeed was oral herpes, you can expect a positive blood test result for HSV-1.)  The red spots don't sound like herpes either--but if you are convinced the spots are abnormal, show them to your urologist.  But accept his assessment, whatever it is:  personal examination by a urologist is far more reliable than anything I can say from a verbal description.

The newer terminology for most cases of 'chronic prostatitis' is male pelvic pain syndrome.  As the name change suggests, it is not clear there is any inflammation of the prostate gland at all, and no evidence the condition is due to an infection of any kind.  See the following thread on this forum from just a few days ago:  http://www.medhelp.org/forums/STD/messages/4380.html.  Also scroll down to the comments on June 1, with another website quoted by another forum user and my comments about that website.  You might discuss the concepts described there with your urologist, including whether the pain from varicocele may have the same sort of origin, i.e. pelvic muscle tension.

Bottom line:  From all you describe, it seem extremely unlikely that you have anything that will ever cause you significant harm, other than stress; and nothing that can ever harm a sex partner.   You may have to just learn to live with your symptoms--but that's often pretty easy once someone finally becomes convinced that nothing harmful is going on.

Good luck--  HHH, MD
