Hi!!!  I got your response about my "Cushing's Symptoms" being PCOS...I've had my estrogen levels checked and they seem to be fine...I also had a TAHBSO in 2000.  I went into Premature Ovarian Failure at 28, my reproductive endo doc never mentioned PCOS.  I'm off to the Endo Doc here in NJ tomorrow but I'm wondering what is the #1 test for cushing's and how long do the reults take to get,  if it's this 24hour urine....do you need more then one sample (ie: x3  24 hour samples) and how reliable is a serum cortisol level...mine is in range but doesn't that just look at "a moment in time", not the whole picture???   I am concerned that I really have some endocrine issue that is causing all my wacky symptoms...first the POF, HTN, High Triglycerides, Insulin Resistance, SOB, Fatigue, Osteopenia, weight gain, Fatty Lump on Upper back and neck, the list goes on and on(a healthy person doesn't have somuch wrong with them)  My throid levels seem fine and my prolactin, CMP and  CBC are good... triglycerides 400, A1c 6.4....  Any input would be great...  Jen in NJ

Still sounds like PCOS with insulin resistance -- or simply an insulin resistance syndrome.  24h urine cortisol is where I start -- if first is normal and I am suspicious then would do another.  A random cortisol blood level has little value, whereas an 8am level having taken 1mg of dexamethasone at 11pm the night prior (dex suppression test) is valuable -- a normal response is a low cortisol.
