Redness in Tip of Penis with Burning Sensation

I have had regular encounters with escorts over the last several years.  I always use a condom for intercourse but do perform oral sex on most of the girls and have them perform oral sex without a condom on me. Since my last encounter about 5 days ago I have developed a swollen opening to my penis and it is red and inflamed and somewhat painful.  There is also pain at the tip while urinating.  Upon urination at one point my stream was split up due to the swollen gland.  I visited a local walk in clinic and they took a urinalyis and looked at my penis.  There are no signs of discharge and the urinalyis was normal.  The Dr. said it was likely a fungal infection.  He did not (nor did he test by swabbing or whatever would typically be done) think that I had any STD.  He said that it would be very unlikely that I could contract an STD with oral sex.  Again, I used a condom for intercourse.  Do you have any ideas what this might be?  What additional tests might I take for STD's?  What are the risks of disease transmission through oral sex?  What does a urinalysis show?  Considering I always use a condom do you think I a, at risk of getting HIV?  Please help.

To answer your questions:

1) I would certainly test for gonorrhea and chlamydia - it can certainly be spread via oral sex - although typically it affects the throat. This should be documented negative before excluding it as a cause.  I would also consider herpes since this is one of the more commonly spread diseases.  HIV can also be passed via oral sex - although it is rare.



2) As for additional tests, I would test for herpes (there are serum tests), HIV, syphillis, and gonorrhea/chlamydia.

4) A urinalysis would show evidence of genitourinary infections (via the presence of WBC's), but it may not be positive in every case.  



5) As mentioned above, there is a rare possibility that HIV can be transmitted via oral sex.
