Urethral stricture causing kidney problems?

I suffered a fractured pelvis in an RTA 2.5 years ago (I'm now 28), which resulted in a partially ruptured urethra. The rupture was treated with catheter placement for just over 3 weeks. Since then, I've had a weak stream and the usual symtoms of a stricture and was followed up 10 months later with a uroflow and ultrasound for PVR (4.1 ml/s and ~200ml respectivley)-the stricture was confirmed (3 months later) with a urethroscopy and VCUG (only .5cm long but very narrow). Ever since then I've been waiting to see the urologist to find out what he plans to do (I live in Britian so these time-scales are normal for our overstretched NHS). Recently though, I've developed some back pain around both kidneys when I urinate and on one ocassion, when out for a few beers, I had the most horrendous pain in my kidneys which only started when I began urinating! This can also happen if I drink lots of much water. Is the delay in treatment causing any adverse effects to my kidneys? Luckily though, I haven't had any infections but still feel full after going and have severe symptoms. I also take 150mg Diclofenac daily for residual hip OA. Your insight on this would be greatly appreciated.

Any type of stricture - whether it is a stricture from the urethra, or blockage from the prostate can lead to hydronephrosis (swelling of the kidneys) as well as kidney failure.



Because you are at risk for this I would suggest obtaining simple blood tests to evaluate the kidney function (via the creatinine level) to ensure this isn't the case.  



Hydronephrosis can be easily measured via a kidney ultrasound.  If you are having back and flank pain in the setting of a urethral stricture, this may be something you may want to consider.  



I would discuss these options with your personal physician.
