Can holding back an ejaculation by gripping the penis cause blood in the seman

It is possible but unlikely that holding back on the penis can lead to blood in the ejaculate.

Many cases of blood in the ejaculate are benign, but infection and prostate cancer would be some of the causes that would warrant attention.

You can consider a urine test for infection as well as STDs to evaluate for infection.

I would also examine the prostate to evaluate for possible cancer.  A transrectal ultrasound can be considered for a more comprehensive evaluation.

You can discuss these options with your personal physician.

Followup with your personal physician is essential.
