cystoscopy concern

I am a 37 year old non smoking father of three. I have recently had a small amount of blood detected in my urine. The result of this is a precautionary cystoscopy to be performed on Friday under local (although I was offered a general). I have had some very mild kidney and bladder area discomfort recently but though I was just getting ‘run down’. Also I have suffered a nasty ‘cramping‘ pain in my perineum. It is at the base of the penis before the rectum. This normally occurs after urination when you apply your ‘stop valve’. I have not had the pain after intercourse/ ejaculation although this is a concern. The cramp can be alleviated by relaxing the area and the pain subsides. Whilst this has happened only occasionally over the last few years I recently got the pain whilst sitting down and not connected to urination. This time after the pain subsided I was left feeling uncomfortable for some days in that area. It is as though the muscle cannot relax fully and is waiting to cramp. I have tried to explain this to the doctor but not been given any explanation. I have been under a lot of tension recently and find it hard to relax.

My concern is that having the cystocopy will be painful and trigger the cramp whilst inside. I assume the scope passes that way. I cannot find anyone with similar symptoms to ask and my nervousness is adding to my tension (and probably my pain). I am not sure what part of the body is affected but it is my urination ‘shut off’ or ejaculation valve/muscle. Am I right to have a local? Should I be sedated? Will they damage me if I am asleep? I also appear not to have the flow control I once had although this could be in the mind. I have never had my prostate checked but I guess this will be done on Friday?

Please can you indicate what might be wrong and what to expect on Friday. The cramping is a great concern as are the possible findings and I am thinking of cancelling although this would be foolish. I do not wish to make things worse.

The prostate should certainly be checked - any infection or enlargement can be leading to your symptoms.  This can be evaluated for by your urologist.  



Cystoscopy is typically done with local anesthesia.  If you are alseep, it would be unlikely that permanent damage can occur.  



Without knowing what is causing the cramp, it is difficult to speculate whether the cystoscopy would trigger it.



However, I would certainly discuss this concern with your urologist prior to the procedure.



Followup with your personal physician is essential.
