Methadone detoxification and how do you find a qualified Dr. in my area.

I have been on drugs since a child. I am also manic depressive which I started self medicating as a child either by over the counter medications or by sneeking prescriptions meds from home. My choice of drug for the past 6 years has been opiates. I have been to drug rehabilitation for drugs 3 times. I have now contracted Hepatitis C. Morphine and Dilaudid are my choice now. Last time in treatment the detox protical was 5 days. Upon leaving treatment I was still having physical withdrawal sytoms. I have tried to quit but all in vain even with support groups. I am being treated for manic depression but it is still a battle I can't handle combined with opiate abuse. My daily dosage of morphine is usually from 500 to 1000 mg. per day. I have thought of methadone continuous treatments or a longer detox period. How do you find a M.D. or Psychiatrist in my area who may agree to this treatment. The closest methadone clinic is over 100 miles from my hometown. Since my health is starting to deterioate what have I got to loose.
