Be there to support your brother, and ask him to read this forum. Seeing how many others there are like him may help him to want to stop. Also, if he reads this I hope he realizes the longer he goes on, the harder it is to keep getting enough painkillers, meaning he will either start forging, shopping doctors, etc. Eventually there won't be enough pills and he will hit rock bottom. Better to stop now before that happens.
With respect to how you can tell if someone is on painkillers, the best way is to look at their pupils. The pupils will be constricted (very small), even in low light. Best of luck to you.
We have had an intervention with my brother today. He was not very happy about it at first. He stated he had already planned to stop. We have set up a plan to slowly reduce his dosage. He is going to start at 7 tablets/4x day for a week, then 6-4x/day @week going to 4-4x/day @week and so on. He has refused to voluntary commitment to detox, but has "promised" that if he strays from this plan that he will go for professional help. I hope, but don't think he will do it on his own, but the family & his friends will support & help him to try.
As for telltale symptoms, he is actually pretty normal in his day to day actions, so Kate, take Brians advise & look into your brothers eyes. That is the one thing I noticed about my brother. Good Luck.

I went on methadone, the average daily dose is 65 mg
I started on 15 and am now on 11. I know it sounds
heavy but at least he can get himself together and come off
slowly
Regarding the methadone. I went to a meth clinic on Monday this week. I'm taking 8-10 vicodin a day. The didn't actually REFUSE me, but said that since I'm in cronic pain, they didn't feel the methadone would help since once I've detoxed, it's a catch 22 because of the pain. It took me 4 weeks to finally get the nerve to go there and now I'm turned down. How is it with the meth? Does it really help the withdrawl? I'm sure if I go back and tell them I want it badly enough, they'll help me. But I'm a little scared of meth on the other hand. Let me know please..
