Complicated EP problems! Need answers???

by ccu nurse, Apr 28, 2003 12:00AM
I am a 27y/o female having severe problems for last 3 years. Have tried everything and need advice on what would be possible to do next or who I could see there. I have had total of 4 ablations for junctional tach, inaprropriate sinus tach, and left sided atrial ectopic tach. I do have a dual chamber pacemaker. I did get some relief from last ablation for awhile and I've not saw the junctional tach anymore, but over the last few months my heartrate has barely been under 120's resting and when I'm moving around 170-200. I am unable to work or do anything. They have not considered another ablation this time because I suffered from a brainstem stroke after the last ablation. Also they said my SVC was extremely tight and they could barely thread a wire before(so they were concerned about more complications). I have tried all antiarrythmics: Flecainide seemed to help the most but I had sustained V-tach and it was discontinued(which they said should not happen in the structurally normal heart). I also have severe uncontrolled hypertension and suffered from hypertensive encephalopathy a few months ago.I have alot of severe chest pain and don't know whether to worry or not? I am not overweight and do not smoke or drink.I have been in the hospital for the last 2 weeks and am taking Sotalol 160mg BID, Tenormin 100mg BID, Cardizem CD 240mg BID,Hyzaar 50mg BID,Catapress TTS patch 0.6 q weekly, and some PRN meds(alot of meds, huh). I have been checked for everything and have seen endrocrinoligist to rule out any pheochromocytoma and had a renal angiogram which was normal about 6 years ago.

Dear CCU Nurse,

Sorry to hear about the medical problems you have experienced. Your case is very complicated and one that needs a careful review before making a comment. We see people with complex cases such as yourself here in the electrophysiology clinic. I would recommend that you make an appointment with an electrophysiologist at a lare university based medical center such as the Cleveland Clinic Foundation. You can visit our website at www.ccf.org or call 1-800-CCF-CARE to schedule an appointment  with an electrophysiologist. Sorry I can't be of more assistance, but in a complex case such as yours I would rather say nothing than to offer an inaccurate opinion.

Thanks for your question,
