I am 43 yr. old. My medical history: LUPUS, COPD, HYPERTENSION , DIABETES. I have chest pain and my recently cardiac tests: Cardiac catheterization in October 2004, was normal (only 33% blocked) e no procedure done. Nuclear cardiology stress test: 06/2005: Left ventricular ejection fraction of 57%. Echocardiogram 07/2006 the results were as follows:
Diastolic LV dimension 4.72cm
Systolic LV dimension 2.95cm
Intraventricular septum in diastolic 1.29cm
Intraventricular septum in systolic 1.80cm
LV posterior wall 3.53cm
Aortic diameter 3.53cm
Left atrium 4.05cm
LV fractional shortening 39%
LV ejection fraction 7.0%
LV mass 266grams
1. There is mild systolic dysfunction and mild left ventricular concentric hypertrophy.
2. Doppler color shows mild regurgitation mitral valve and mild calcification in the mitral valve with left atrial enlargement
3. Aortic valve shows mild calcification and trivial regurgitation with mild sub-aortic stenosis. There valve measures 1.45cm2 gradient of 7.6mmH
4. Tricuspid valve shows trivial regurgitation .Tricuspid valve shows trivial regurgitation
Medications: Laxis 40mg, Zestril 10mg (twice day), Hyzzar 100 – 25, Imdur 30mg (twice day), Coreg 12.6 mg (twice day), Glucotrol XL 10 mg, Singulair 10 mg,advair,Prednisone 20mg, Prevacid 30 mg. Nitro 0.4 ( as needed)
My diagnostic is aortic valve stenose, aortic valve regurgitation or other? My cardiologist increases coreg to 25mg twice a Daly. My family doctor want I make MRI (chest – heart) r/o pericardites, and have a second cardiologist's opinon. I'd appreciate any info. Thanks

Those valve findings are not very significant in the spectrum of severity. It is important to know if ouve had any disease such as rheumatic fever in the past, or any family history of hypertrophic cardiomypathy. Overall, the findings require regular followup, consideration of antibiotic prophyaxis with procedures, and strict control of your cardiac risk factors such as cholesterol and blood pressure.
A valve can have some degree of stenosis and regurgitation at the same time. It sounds like your aotic valve may have both. Lupus can also be associated with all of the cardiac findings you mention.

I’m glad those valve findings are not very significant and serious! But, yes, a had rheumatic fever when I was teenager (14 until 20 years old), and, my grandmother and two aunts died, because a hypertrophic cardiomypathy and they had angina all ther lives. My mom has hypertension and high cholesterol.
My doctor makes my follow-up every 3 month, and, I have appointment for MRI (chest – heart) r/o pericardites.
