Burning Sensation on Scrotum Update

Dr. R,

Below, for your reference, is the question I asked in early March. Currently my scrotum can tolerate nothing but pure Vaseline. The good news is that over the past four years the color has almost returned to normal. But I have a constant itching/burning patch about the size of a dime which won’t go away (located on right anterior). My question today is if it would be OK to use a mild topical steroid on it. I’m fearful that the original redness/burning was cause by an overuse of strong topical steroids, and I certainly don’t want to do additional damage. But perhaps a low strength steroid (since I haven’t used any topical steroid in four years) might offer some relief? If so, what low strength steroid would you suggest… either OTC or Rx…

Dr. Rockoff,

For 30 years I had struggled with ongoing groin rashes. In 2000, I used 200 mg Diflucan one per da. for six months, which cleared up the problem very effectively. I now use one 200 mg Diflucan tablet once each week to assure no return of the problem. I have been rash free for four years now.

My problem now is that I created a sensitivity on my scrotum by overtreatment of cortiosteroids (including Temovate) as well as Castellani’s Paint. While the color on my scrotum has slowly returned to almost normal from a reddish color, a sometimes mild, sometimes severe (but always present) burning and sensitivity remains, predominately on the right anterior side of my scrotum. Application of Protopic, Elidel, and Emu oil seemed to aggravate, rather than help, the sensitivity.

My dermatologist has recommended that I try Prax lotion (pramoxine) and/or Eucerin Itch Relief Moisturizing spray (menthol 0.15%).

I’d like your opinion as to these two topical applications re, the scrotum, i.e., are they likely to reduce or increse the sensitivity?. Also, for me to consume internally, what do you think of hydroxyzine, alpazolan (Xanax), and/or Neuronton—would any, or a combination help, and if so, what dosage would you recommend.

Finally, is there anything else you would suggest, either internally or externally?

I like Prax and use it all the time for itching. However, I think it will just cause burning and sensitivity on your scrotum. Given everything you've used, I would advise that you do as little as possible. Just apply a bland lotion like Lubriderm and use nothing medicated at all. As for the burning and sensitivity, I advise you to train yourself to ignore it. In time, it will subside, just as the redness did. Steroids, however strong, do not cause permanent damage.

It is OK to use a mild steroid on a localized area.  Just check to see whether it helps--it should do so in a few days.  If that happens, then just use it from time to time as needed.  Other options are Protopic and Elidel, two non-steroid creams used for eczema and inflammation.  But first see if the steroid helps in the short term.

Could you please define "mild steroid"? OTC hydrocortisone 1% or an Rx?; if Rx, what specifically-- I was thinking of Desonide 0.05% ointment-- would that be OK or too strong? Thanks Ed S.
