Bumps at corners of mouth

I have an annoying chronic problem that I've seen a derma for but he can't seem to figure out what it is.For the last couple of years I've had outbreaks that are pimple like, at times, but, also I have "raised areas" that the Doc calls "inflamed"(even though they are not red or even sore. They can be a few in a row, on an angle form the corner of my mouth. They are almost like scar tissue, in that they are deep bumps. They can last months. The pimple like ones are more on the surface, can have a small whithead(but not be filled with anything), and they can go away faster. For these I've treated them like acne, but sometimes that doesn't work like it would when I have normal acne( which I don't have very ). I've been prescribed an atibiotic, which didn't work, Nystatin, cortisone creams, and some of my own homeopathic remedies. It's really frustrating. It's recurring and almost always present, to one degree or another.
Do you think this is serious, or some kind of fungal infection? It also seems like it's coincidentally not just one issue.
Any ideas on a diagnosis, or maybe some specialist for this?

These certainly don't sound like any kind of infection, fungal or otherwise. Perhaps these are some form of blocked pore. Treating these can be quite hard, and sometimes the best thing is a facial, even though that just minimizes bumps and doesn't prevent them. But it seems to me that your dermatologist has the right idea and the right approach.
