For about a month I've had an ache / cramp in my rectum off and on. It usually feels OK 1st thing in the morning and then is worse towards bedtime. I have no other symptons, everything is normal, no blood, bowel problems etc. Basic blood tests showed nothing abnormal, also PSA is OK. I've had some basic hemorrhoids on the outside but nothing major according to my doc. Could this be stress related or something structural gone wrong? I work out at the gym often but am also under considerable stress.

Dear Jim,
You do not provide your age, a fact that would influence the probability of the various causes for your symptom. When investigating a complaint of rectal pain, possibilities to consider include rectal fissures, hemarrhoids and less commonly colon tumors. If your symptoms persist, you should see your physician who may do a flexible sigmoidoscopy (insertion of a telescope in your rectum).
