Hi Doctor  I am a 29 year old male I have a problem that has been irritating me for the last 3 months.I was on a high protein diet which included protein shakes , bars ect.After a 2-3 months later I become constipated with a anal fissure (diagnosed by a doc)and I also had blood test done because I had serious muscle twitching in my calves and left arm and sometimes my thighs. The muscle twitching started more or less the same time and occurs when I sit. The blood test showed that I had to much urea and creatine in my blood. So I stopped all the protein and drank a lot of water.I also started getting serious lower back pain mainly on the left side of my lower back.I tried stool softner but it did not work. When it started I did not pass a stool sometimes for up 2 days. I felt a lot of discomfort in abdoman mainly under my left rib cage.It feels as if there is a ball which is a liitle painful under my left ribs and gets worse when I went to gym Also when I sit for to long my stomach region starts to hurt .Its does not feel muscular. I am not constipated any more but it seems if there is a large amount of ***** discharge after I pass a stool and the stool seems to be very pale.I dont know if it is from the fissure or not. What I would like to know is , is it possible that I can get imflammation in my intestines from being very constipated or can I get some sort of infection from a deep fissure. Is it possible that imflammation or infection can cause muscle twitching.My heart tends to race even if I am not doing anything physical which it never did before. This has been going on for the last 3 months and its very frustrating because I cant do anything physical. Thank you

Difficult to say without examination.



Although possible, I am not aware of constipation in itself leading to colitis or inflammation.  It would also be atypical that such an infection could have arose from the anal fissure.



If there is continuing constipation a lower endoscopy would be helpful - either a colonoscopy or flexible sigmoidoscopy.  This can evaluate for colitis, or any anatomical abnormalities leading to the symptoms (i.e. a mass or polyp).  



More specific studies like colonic motility, defacography, or anal sphincter studies can also be considered in the workup of chronic constipation.



If the symptoms continue in the face of normal testing, you may want to consider treatment for irritable bowel syndrome.



Regarding the stomach discomfort, you can consider testing like an upper GI or upper endoscopy to rule out diseases like GERD, an ulcer, or inflammation of the upper digestive tract.



These options can be discussed with your personal physician.



Followup with your personal physician is essential.
