if anyone has experience with a patient experiencing
rapid progression of paralysis with evidence of
demyelination and axonal degeration (paralasis
with grimace response only), what is the prognosis
for recovery?

Sorry Char but we really need more information on this case. I assume that you are referring to a peripheral nerve problem. Some disorders such as GBS (Guillain Barre) can cause a demyelinating problem which progresses rapidly and may lead to complete paralysis - most of these patients will then go on to recovery. If most of the injury is axonal then the recovery is not as good. If you can provide us with more details then we can provide a more detailed answer.
