Topamax and Estrostep intercation

I know that Topamax does not affect the clearance of noethindrone, BUT it does effect the oral clearance of ethinyl estradiol to a certain extent.  My wife is taking 75mg/day of Topiramate (as a migraine preventative - Dr. Diamond prescribed)and the Estrostep dosage is 20 mcg, 30 mcg and 35 mcg of ethinyl estradiol.  



Is this dosage of Topiramte low enough such that the clearance of ethinyl estradiol is not affected, or are their signs we should look for (i.e. break through bleeding) that would indicate the efficacy of birth control is being comprimised?

Dear Ryan:



You are correct and Topramax does effect OCPs.  We usually recomment that the patient go on a mid dosage OCP or change the birth control method.  One can follow whether there is break through bleeding.  I would talk to your wife's OB and get their input about dosing.
