Dear Jay:

Sorry to hear about your epilepsy. Yes, one can have only auras without subsequent motor seizures or loss of awareness.  One can also have simple motor or sensory seizures where the seizure activity occurs without loss of awareness.  The International League Against Epilepsy classification calls these types of seizures simple (there is no loss or change in awareness).  We think that aura are the beginning of seizure activity.  In the cases where there is an aura and no overt seizure activity, the likely explanation is that there are a group of neurons in an area that are activiated that produce the aura.  However, they do not spread to other areas that produce motor or loss of awareness.

Therapeutic levels are usually given by the lab that does the testing.  By our lab a level of 10-20 of dilantin is in the therapeutic range.  However, these values are only relative in that whatever the dose of medication that stops seizure activity without side effects (causing medication withdrawal) is the therapeutic value for that particular patient.

I hope that I have helped some.

Sincerely,

CCF Neuro MD
