Assortment of symptoms... neural? Musclular? Confused

I am 31 y/o who received an injury in Oct. on a ropes course jumping from a telephone pole to catch a trapeze (which I did only to experience sever neck pain 40 minutes later that lasted 4 days)that later exploded into a collection of parasthesis in my shoulders, bi&triceps, neck&lower back pain. I was referred to neuro who did exam (all things were normal except DTR were +1 bilat elbows and wrists (+2 last exam 3 wks ago). Lower body was +2 bilat. All strength were 5/5. Went through cervi MRI that, as my neruo explained, was "normal". I was followed by neuro for 18 wks. Saw neuro 3x, no signs of any neuro concerns. DX of neck/shoulder sprain, myofascial pain&anxiety. There have been intermittent and very short lived fasiculations at shoulders, but neuro dismissed as benign. As a result he decided not to do EMG. Referred to PT which did a great job while it lasted but is no longer, nor is neuro. I have been done with PT for 3wks and still have intermittent parasthesis in upper body, especially when overdoing home PT exercies. However,I have since been doing lower body PT stretches that have resulted in muscle pain at knees and when using leg muscles too much get more pain and on 2 occasions have had fasciculation (brief and intermittent)in my lt quad over the knee no stregth loss. PT sez it is from overuse, esp since I have been dormant phys since the injury. Questions: 1)how long general for things to get back to normal? 2)What is deal with fasciculations? Worried about ALS due to them even though no wasting or weakness. 3)Burn/stiffness normal with quad strain?  Thx

Your story does not suggest ALS. Fasciculations can occur due to overuse of your muscles, electrolyte abnormalities, and a condition called benign fasciculation syndrome. If these fasciculations are secondary to overuse, then mild re-introduction of activity would be recommended. As a general rule slow or non-weight bearing excercise to start with for a few weeks, and then advance slowly. Regardng the paresthesias, some conditions such as inflammatory conditions or connective tissue disorders can sometimes present with these symptoms. You can discuss this with your regular doctor. Also, anxiety and stress can cause many different symptoms. If you are still concerned about these symptoms you should discuss them with your neurologist and if needed obtain a second opinion.
