Nodular Scleritis, Face Pain, Headache, Ear Pain

History:  43 year old female.Diagnosed with Left Eye Nodular Scleritis 9/01/04 - responded to treatment with nodule size, redness & boring pain decreasing (Prednisone 60 mg, Prednisone eye drops, Ibuprofen).9/03/04 - developed excruiting, pulsing, electricuting, burning right side face, cheek, upper jaw, chin, and right ear pain - unrelenting, worsens approx. 2 hours upon awakening and continues day and night without relief.Heat & cold hurts it.Looking up (eyes up)causes face and eyes to hurt. Ears tested - perfect hearing, no infection.Cat Scan brain and orbits with and without contrast - negative.Bloodwork for Lupus, RA, Wegeners - negative.Positive Romberg test. Loosing balance/falling since 9/03/04 - no spinning or darkening of room, just fall when standing or walking like legs give out - fall backward or to the side.No unconsciousness with falls.Appendectomy 1981, T&A as child, Cholecystectomy 1998, Cardiac Ablation 2001 for Wolf Parkinson White Syndrome, Endometrial Ablation 2003, Left Knee Repair (ACL, MCL) 2003. Pellet right eye (30 year old injury).  Controlled Hypertension

Medications prior to 9/01/04 - Uniretic

Medications now:  Prednisone 20 mg, Prednisone eye drops, Valium 15 mg, Ibuprofen, Hydrocodone prn pain

Family History:  Mother MS; parents hypertension, Grandmother diabetes, Cancer - every kind imaginable in family (cousins, aunts, uncles).  

Question:What's happening? What tests do I need done? The Opthamologists and ear doctors do not know what this is.

What can I do to get the pain & falling to stop? I live in SC where should I go for treatment

Without examining you and reviewing your history, I can not provide a diagnosis. The initial face pain could have been a form of trigeminal neuralgia. However the balance problem would not be explained by this. You should have an evaluation by a neurologist. A MRI of the brain with contrast to look for signs of inflammation would be helpful, and you may require a spinal tap after this. The spinal fluid can be analyzed for signs of inflammation. Depending where you are in South Carolinia, you could go to the University of South Carolina, Duke University, University of North Carolina, or Georgia. In addition to these tests a more comprehensive connective tissue and inflammatory evaluation could be completed. Good luck.

Not sure if this is a help.  My mother has this condition and she has been seen by a doctor in Boston at Mass Eye and Ear.  He has been a great help.  His name is Dr Foster.
