I have been trying to find answers for this problem now since 1998 or 99.  It started with a general discomfort and lack of stamina mainly in the larger upper leg muscles and back.   The early symptoms were accompanied by slightly elevated CK readings,300-400 range. Early tests included EMG and MRI's which showed very little.  EMG showed slight signs of Neuropathy, no signs of myopathy.  MRI results showed slight but not significant degeneration in L3-4 and L4-5, which the neuroligist originally thought was contributing to the problem.  My GP did not feel this was significant enough to be contributing and the notes from the radioligist who read the MRI also felt it was not part of the problem.    In July 01 I felt terrible, mainly muscle and back pain, and the only test result was elevated CK numbers, 874.  The Rheumetoligist I see for Goute ( goute has been under control )recommended a muscle biopsy.  The results were at best inconclusive.  I have problems doing simple things like walking around the Mall or grocery store. Repeated physical efforts cause pain in legs and back. I recently returned to the local neuroligist and reran the EMG.  He reports it now shows neuropathy and slight myopathy.  He calls it aninconvenience, but it impacts my life much more.  I am taking 400 MG Neurontin and muti-vitimans.  I am considering asking my GP to recommend a different neurologist for a second opinion.  Is the a reasonable plan of action.  RTJ

It would be very reasonable to ask for a second opinion. If you do, I would recommend traveling to an academic facility. If you are in the area we have a wonderful neuromuscular section. Without examining you and reviewing your testing it is difficult to come up with a diagnosis. However, the elevated muscle enzymes would incdicate some destructive process to the muscles, possibly indicating a myopathic process. You have stated that you have gout and some of the treatments for this can damage muscle or nerves. I would recommend discussing this with a neuromuscular specialist. Good luck.

im not a doctor. walking difficulty  and muscle wasting can be a sign a sign of  beri beri  check other notes by keys, its a b1 defiency if u havent had b1 levels checked maybe getting that done can help if thats whats causing the weakness in the lower body  its easilly reversed  with  intravenous b1 or shots, to get ur levels back to  normal then they follow with supplementation,left unchecked  it can be fatal if thats what it is.  ask ur general practictioner can check for it , and get an exact dose for what u need if thats it. taking b1 by itself can cause disturbances with the other bvitamins, so ask ur doc,

a friend of mine had that and thats how it started she wound up in a wheelchair from it. fatigued and walking problems

muscle pain and weakness with fatigue are the first symptoms some of them of b1 defiency.difficulty getting up and walking. are next , atrophy in the muscles generally effecting lower body
