CSF and MRI Results

by racquelmorris, Aug 21, 2005 12:00AM
34 yr old Female

Serology:

Myelin Basic Protein: <2.0 UG/L

Oligolonal Bands, IGG: One Band

One gamma restriction band is present in the patient’s CSF but is not present in the patients corresponding blood serum sample. This finding is insufficient to support a diagnosis of multiple sclerosis. Single bands are sometimes seen in patients with b-cell tumors of the brain. This data should be interpreted in conjunction with all clinical and laboratory data pertaining to this patient.

Cell count/diff, CSF

Color, CSF: colorless

Appearance, CSF: clear

WBC, CSF: 10 H/mm3

RBC, CSF: 60/mm3

Glucose, CSF: 54

Protein, CSF: 26

Differental comment: Occ. Lymph Seen, Few Isolated RBCS Noted

MRI wo/w contrast:

A single punctate white matter focus of increased signal intensity is seen on the T2 FLAIR sequence within the mid left hemisphere at the level of the lateral ventricles. This is a nonspecific finding and no other white matter lesions are identified.

Can anyone tell me what all this means? I was being sent for all these test to confirm a diagnosis of MS (having an EMG on the 1st). I have severe leg pain which tends to be worse in the R but sometimes both legs are equal in the pain. The pain feels deep and like my legs are just going to explode (the only way I can describe it). I sometimes get crawling sensation on my legs and arms at night. I only average about 4 hours of sleep due to pain per night. By afternoon time I have to use a cane to get around. I get dizzy and fall or loose my balance. I trip or stumble all the time.

I cannot give you a definite clinical opinion as I do not know your case in detail

Your test results here though do not support any cause for your leg pain

The one abnd in teh CSF is probably not significant in the light of your history and other test results, but a second test in a few months might be indicated to follow through on this. The CSF white cell count is slightly high but is nonspecific and may be related to infection, viral meningitis, other inflammatory processes, again a repeat sample in a few weeks to follow through

If I was your doctor I would like to know what the CSF pressure was (measured from the spinal tap)

The MRI finding is again non-specific, and not suggestive by decription of MS.

What are you doing to treat the pain while waiting on a diagnosis? If your aren't already, I suggest you seek a Pain Management doctor. No sense in suffering endlessly while the puzzle is put together. Hopefully you will get a quick response but I've noticed that it sometimes takes a week for the question to be answered. Good Luck!

I am seeing a pain management doctor but all he will give me is neurontin. He will not take the time to learn about what is safe during bfing and refuses to give me pain med even though my son only nurses 2-3 times a day now and I would only need to take it in the evening after his final feeding. Very frustrating but that is what I get for moving to NM, healthcare here is like third world compared to what I am used to back east.
