Question Title: Sphincter and incontinence

Forum: The Urology Forum
Topic: Incontinence

I have developed incontinence. I have had all the tests and
have been told my sphincter is at 30power. I have had one
collagen injection, I am still leaking. I do the exercises
and have had this problem for eight months. I had bladder
surgery over a year ago, is this all related. I never have any
dry time. Is there anything more that can be done? Also I
have no sensation that this leaking is taking place until I am
very wet.

Dear Shirley,
Thanks for your questions but more history is needed.
Have you been diagnosed with mostly stress or urge incontinence. I am not familiar with the term “30 power” as a gauge to sphincter strength. Does this mean your “ leak point pressure” (the pressure in your bladder at which you leak) is at 30 cm of H2O? Collagen is used to help the urethra stay closed in stress urinary incontinence caused by abnormally weak sphincter. What type of bladder surgery did you have?, a bladder neck suspension?, a pubovaginal sling? These are surgeries for stress urinary incontinence. Do you have risk factors that would predispose you to fail surgery? obesity?, chronic coughing i.e. smoker? If you have failed one of these surgeries in the past you maybe a candidate for repeat surgery. A sling is recommended for a failed bladder neck suspension. I would certainly ask your doctors these questions for a more tailored answers to your particular condition.
