I'm a 48 year old white male in generally good health except
controlled high blood pressure. Recently during ejaculation I
noticed blood tinged semen. On and off for several years I have
had periodic discomfort after ejaculation, nothing painful, just
uncomfortable...no blood in urine or semen. About 3 times a year
I have very strong sensations for bowel movements that are false.
In recent months I have had some sexual disfunction in that in sexual
situations an ejaculation occurs without the benefit of much
stimulation and little or no accompanying sensation...arousals
are less frequent. No problems with urination at all. I, like
many, am scared to death. Has anyone had or heard of these
symtoms? Any advice would be greatly appreciated

Dear Ed,
Hematospermia (blood in semen) is usually a benign, self-resolving problem. It generally occurs in younger men (mean age 37) and goes away within a few weeks. One of the causes for blood in semen is uncontrolled hypertension, which will be something you will want to investigate thoroughly given your medical history. The other possible causes are non-specific inflammation of the urethra, prostate, and/or seminal vesicles. There could also be more serious, but less likely, causes for blood in semen, and your urologist would be able to rule these out. A complete work-up would include a thorough physical exam, blood tests (PSA, etc.), and urine cytology. Some of the symptoms you have described would suggest possible prostatitis, which could cause blood in semen. In reference to your sexual dysfunction, you are describing what sounds like premature ejaculation, as well as possible impotence. Because I do not have the benefit of knowing specifics, you need to consult your urologist who can address this whole cluster of concerns.
