I have a great deal of pain in my left testicle. I have had two varicoceles surgerys in the last six months. Im still in great pain just to the left of my penis, and moves down the testicle and down my flank. My doctor at mayo clinic is going to remove my testicle in one month if my pain consist. Is there anything that you might think would help me, so I don't lose my testicle.

There are various causes of testicular pain, including:

1)Infection (epididymitis);Inflammation
2)Spermatocele
3)varicocele
4)hydrocele
5)tesicular cancer
6)Torsion
7)Inguinal hernia

Given the limited background you present, it seems your problem was caused by a varicocele. The fact that you had a second surgery may suggest that the initial surgery was not fully successful at eliminating the varicocele. Further, chronic testicular pain is not always easy to treat and could persist for years. Surgical removal of the testicle because of pain is sometimes helpful, but there is no guarantee that it will relieve the pain, nor is there any guarantee that the pain won't worsen. A thorough work-up by your urologist would include ruling out any of the other reasons listed above that could contribute to or cause your pain (in addition to the varicocele). The lump in your scrotum may represent a spermatocele which has the potential to cause pain, as well. It appears you have sought other treatment(s), namely nerve injection to relieve the pain, without result. This procedure often helps alleviate the problem that you have described. However, since you say you are still experiencing pain, a second opinion with another urologist (who has the benefit of seeing you in person and doing an exam and reviewing your records) may help you make a decision about the course that is best for you.
