I am 42, divorced, and in excellent physical condition.  I noticed a sore on my penis last Apr, just below the head on the coloration line -- don't know what that line is called.  The mark is whiter than the other areas of my penis and occasionalty gets irritated with intercourse and has not gone away.  I had a lot of skin iritation at that time from intercourse and had experienced frequent small cuts/tears etc.  I just assumed this was a similar.  It does not seem to be raised. Is this possibly cancer?  I have been suffering from impotence for the past 2 months as well...could this be related?



I also have small red/brown bumps on my scrotum. I began noticing these about a year ago, but don't know how long I've had them.  I popped one like one might pop a pimple and it bled profusely and was difficult to stop.  Is this normal?



Note:  I have also had urinary problems (frequency, hesitation, irritation, some pain/burning).  I also have some light pain in the left inguinal area and at the base of the penis -- it seems very localized.  I'm a bicyclist as well.

Dear Jim.,

There are a few things to address here.

First, any non-healing sore,  anywhere and especially on the penis must be evaluated by a physician. Your primary care physician may have to refer you to a dermatologist or urologist for a more thorough evaluation of this “sore”. This may be a simple, infectious or inflammatory process, but malignancy is always to be ruled out. It is important for you to see a doctor about this soon.

Secondly, impotence has many reasons. It sometimes is due to organic causes, and other times may be caused by psychological issues. This obviously needs to be addressed by a urologist. It is unlikely to be caused by the “sore”, but the possible irritation of the penile skin due to intercourse that you experience, could potentially affect your erection indirectly via the psychological pathway.

Thirdly, the brown spots on your scrotum may represent a cluster of blood vessels(“hemangioma”).

These are benign , but would bleed severely if injured or cut into.

Lastly, the urinary symptoms and pain you have described could be due to many causes, but in a bicyclist inflammation of prostate (prostatitis) could occur, and would precipitate the symptoms you have described. Sometimes anti-inflammatory medications(such as ibuprofen) and avoidance of the exacerbating activity(bike riding), as well as sitz baths would help the symptoms.In any event you need a thorough evaluation by a urologist.
