I am a 53 year old male with a weak bladder with the same symptoms as the person Robert who has restricted flow, up 3 to 4 times a night, and every 1-2 hours during the day.  I am not retaining urine, have no blockage, my prostate is no longer enlarged but my flow is very slow about 7... per minute...normal is 12-14 from what I know.



Two specialists have diagnosed this as overly strong sphincter and urodiaphram muslces clamping down on the urethra causing this restriction.  Other than hytrin, no other treatment was recommended.  I do Kegel and bladder exercises and have been taking saw palmetto for over four years.  The hytrin does help at night but causes tiredness well into the next day. Bladder neck surgery is a 50-50 risk with no guarantees.  I am not convinced this type of surgery will help.



Any other recommendations that could help my condition that are less evasive than bladder neck surgery?  Thank you.

Dear Jerry, thanks for your question.  It seems that you are experiencing a condition known as "Lower Urinary Tract Symptoms- LUTS". Your main concern seems to be increased day and night time urinary frequency and poor urinary flow rates.  



Common cause for this condition at your age is benign prostate enlargement.  Other causes include spasm of bladder neck, weakness of urinary bladder, interference with neurological control of bladder and/or narrowing of urethra.



Benign prostate enlargement is the most common entity and you symptoms could be due either to spasm of smooth muscles in the prostatic region or occlusion of urethra due to prostate enlargement.  Usually, both mechanisms act together.



Alpha-blocker drugs usually help the patient.  If you feel tiredness due to Hytrin, you may want to consider some newer alpha-blockers such as Tamsulosin, which have more selective prostate relaxation.  If medications don't help, some times we have to consider endoscopic surgical procedures.



You mention that your bladder is weak.  In order to evaluate the bladder, you need to be seen by an urologist, who will perform certain tests for the bladder function.
