Can UTI/Urethritis burning cause tendancy toward premature ejaculation?

I'm a healthy male 38 years old and I had never had a sexual function issue until several months ago when I started to feel a slight burning sensation in my urethra during sex. This burning sensation would come about when I would approach orgasm and make me "feel" like I needed to go (to the bathroom) and also make if VERY difficult to prevent orgasm. Normally, in my past, as I would feel the onset of orgasm, I would stop and slow down a bit, let it pass, and this would enable to me continue sex for long periods of time without that strong, uncontrollable urge to orgasm. I have had treatment for urethritis in the past with antibiotics and I was diagnosed once with Chlamydia a long time ago (can't remember when, but I believe it was over 5 years ago). I also currently do experience some burning (but not all the time) during urination. What is most noteworthy though is the burning sensation during sex does not approach until I believe some slight (leaked) seminal fluid reaches my urethra. It's very evident that this is exactly what is going on. I have not discharge or any sensitivity in my penis or urethra under normal conditions (not urinating or not having sex). Also note, I have had some unprotected sex with multiple partners over the past 6 months, so I may have contracted something new.
Recently, I've been treating (myself) with doxicycline antibiotics, but a 7 day course did not seem to help the problem. No, I am not under a doctor's care as of yet...please don't scold me too harshly!
What do you think may be wrong with me that the doxi cannot seem to resolve? I've read some of your other notes and I saw the issues of other possible problems (STDs, Chlamydia, etc.). However, I thought I would ask again mentioning my specific symptoms to see if I can get a more specific answer. I intend on taking whatever you recommend to my doctor for further tests and treatment. I greatly appreciate this.

Hello - thanks for asking your question.
Doxycycline only treats one type of STD (Chlamydia). You need medications to treat possible gonorrhea, such as Rocephin or Cipro. I would re-test for Chlamydia and gonorrhea. There is also the possibility that the infection may be resistant to doxyclcine.
There is also the possibility that your symptoms are not caused by an STD. Your urine should be tested and cultured for any infection.
Prostatitis should also be considered. Urine and prostatic secretions should be tested and cultured before and after prostatic massage. If present, bacterial prostatitis often requires 4-12 weeks of antibiotics.
Epididymitis can also cause burning - this can be evaluated via the urine and a scrotal ultrasound.
I would discuss all these options with your personal physician.
Followup with your personal physician is essential.
