I have a very odd problem the majority of my life.(38 yrs old now, no health problems) I have a hard time reaching orgasm and ejaculation. My Penis isn’t extremely sensitive, its not numb or anything but I don’t believe it is as sensitive to touch as other men’s might be, or least I have come to believe from what I read and heard. (like I do not have any spot on my penis that has more sensitivity than another, with the exception of the head glands, I.E. I seem to lack that spot underneath everyone seems to know about that is so sensitive) I could never bring myself to a point of orgasm with dry masturbation as a teen and early 20’s. If I use a lubricant I could, but it takes quite a while and often I would tire first or loose the erection trying.
I don’t lack a sex drive in any sense of the word, I can’t seem to go more than a couple of days without getting fidgety for sexual relief of some kind. If I abstain for a week, it gets easier to reach an orgasm, but I still will have to fight for it, but not so long.
This has only gotten worse over time. As you can imagine this has caused strain on my marriage, my spouse often tires before I can get to that point making love and ejaculation. I have come to realize or least believe it is a physical problem than a psychological one, since even masturbation is an issue.
I have a very understanding wife, but I want to know what is wrong, but I am afraid and humiliated to physically go to an urologist for such a odd problem. I did ask for Viagra from my regular doctor in hope it might help. Actually it worked great, it was like suddenly all the nerve endings in my penis became super sensitive, and I was able to achieve a full erection instead of a so so one, that did not go away. Suddenly I could feel
everything , it was amazing, and it was wonderful, and an orgasm could easily obtained, sometimes as little as five minutes. Which would be great, with the exception my wife finds me uncomfortable on Viagra, I am longer and harder than I can normally achieve and this causes her discomfort even with extended foreplay, though I am a typical six and a half or so inches. She wants me to get over my philological issues (she thinks its in my head) and beat this mentally, that I don’t need the medication and it’s a crutch. She also says I am more aggressive in love making on the medication and I guess she is right, but for the first time it feels so good it is very hard not to enjoy the freedom of it. Have you ever heard of a penis that isn’t very sensitive, or my problem? I am circumcised rather tightly, I am not sure they may have injured me back then that caused this. I don’t believe this is in my head, but something else, but I have never seen an article about this anywhere.
What does Viagra do that increases my sensitivity so much, I thought it only helped in retaining erections?

Penile sensitivity varies between patients. It can also be affected by various disorders that affect the nerves - such as various causes of peripheral neuropathy and diabetes.
Viagra works by enhancing smooth muscle relaxation, allowing the penis to be filled with blood easier. To my knowledge, it does not have any direct neurological effect where penile sensitivity is increased.
If there is further concern, you can consider a referral to a urologist or neurologist for further evaluation.
