Cystoscopy..to diagnose a stricture, yet can cause a stricture? Other tests?

by Mitchy, Sep 17, 2004 12:00AM
Dear MD,

Seven months ago (in Feb) due to hourly urination problem my URO did a Cystoscopy on me….he found it to be absolutely normal….since that test, gradually over the course of the past  seven months my symptoms have gotten much worse (have pain with urination, still hourly urination, diminished flow, sometimes a spray or split stream…..in seven months I have been to 4 urologists, have been tested for STD’s, prostate infections, have had DRE’s..you name it….all negative, have PSA .63 (very normal), all bladder ultrasounds POST VOID Residual, normal.  I even did two courses of antibiotics despite negative infection results which still didn’t help…so I am convinced there is no infection at this point…

When I had my Cystoscopy, right after, I was bleeding (from the urethra) post urination and in severe pain for three days afterward.   Two urologist said “that’s an unusual reaction”….. but I still had it, so my question(s) are:

1) COULD THE CYSTOSCOPY I HAD SEVEN MONTHS AGO, HAVE CAUSED SOME KIND OF SCARRING OR STRICTURE THAT IS NOW IN MY URETHRA CAUSING ME, NOW,  MORE OF A PROBLEM THAN WHAT I STARTED WITH? (I have read online in reputable sources, that the Cystoscopy or any catheter, can cause a stricture)

2) IS THERE ANY NON-INVASIVE WAY (ASIDE FROM URINE FLOW VOLUME TESTING—WHICH IS NOT DEFINITIVE), TO SEE INSIDE MY URETHRA TO DETERMINE IF I HAVE ANY KIND OF STRICTURE, SCARRING OR FIBROSIS?   (ANY KIND OF IMAGING, THAT DOES NOT INVOLVE A CATHETER?)

Thank you so much for your help.

To answer your questions:

1) It may be possible that any scarring form the cystoscopy can lead to the symptoms of a stricutre.

2) Cystoscopy is the best test.  You may also want to consider a retrograde urethrogram - less invasive than a cystoscopy, but still needs a urinary catheter for the procedure.  You may want to discuss this option with your personal physician.

Followup with your personal physician is essential.
