Prostititis

Doctor-

I'm a 33 year old healthy male.  About 3 weeks ago I started to experience pain in my rectum and urethra, along with frequent urges to urinate.  I went to Urgent Care and was diagnosed with Prostititis.  They tested my urine and did a manual prostate exam. I was prescribed Cipro and another medication which I can't remember, but I was supplied with a week's worth.  The symptoms seemed to be getting better by the end of the week, and when I went to see the Urologist for my follow up, he performed a manual prostate exam and said it was normal size, and that I should be fine.  The next day my symptoms returned, my medications had been completed.  I should mention that I have also been constipated, and that I started supplementing with Saw Palmetto, Benefiber and Flax Oil during the first week of symptoms, which I have continued to take.

Since my symptoms returned, I went in to see a general practioner and he just asked me the usual questions, and did no exams of any kind.  He told me my Prostitits had returned, and prescribed Cipro.  Since seeing him, my symptoms have lessened a little bit, and some days are better than others.  But, the last few days I have been having the urge to urinate more than normal, and the pain in my urethra has increased.

My main problems are the frequent urges to urinate, weak flow when I do urinate, feeling like I haven't completely emptied my bladder, constipation, pain in my urethra, pain in my rectum/prostate, fever/headache, waking up frequently to urniate, cloudy urine.

Are they missing something here?  Do I have a bladder/UTI infection as well?  What tests should I have done?

I'm getting depressed as this keeps me from doing things I normally do (kickboxing, going out with my wife, etc...).  I have no desire to sit in discomfort at a restaurant or make my condition worse with rigorous exercise.

Please help!

The antibiotics you took would treat any bladder/UTI as well.

I would further evaluate the prostate.  A prostatic massage can be considered - and fluid expressed can be sent off for analysis and culture.  It may be possible for the bacteria to be resistant to the Cipro.

Another consideration would be a prostatic abscess - which would require drainage.  A transrectal ultrasound can be considered to evaluate this possibility.

These options can be discussed with your urologist.

Followup with your personal physician is essential.
