Interstitial cystitis

by gowinmktg, Jan 19, 2007 12:00AM
Dear Drs,

I am 25, male, about 1.5 year ago, I noticed everytime i drink coffee/tea, I get this discomfort around my pelvic area, sometimes around my testicular area as well, and I get really frequent urge for uraination.  To sum up my symptons.. I have.

# ower abdominal pain,

# urinary frequency,

# decrease strength and caliber of the urinary stream as well as

# pain in the testicles or perineum (the area just in front of the rectum and behind the scrotum) especially after coffee/tea.

I DON'T HAVE:

# cramping,

# bladder spasms,

# burning during urination,

# difficulty in beginning urination,

# interrupted urination.

I didn't think it was a big deal, until I felt that my erection was not as good as it used to be, so I went to see an doctor.  The doctor did a test.. basically put a microscope in my penis(uroscopy?) and came back with the result that I have interstitial cystitis, which only 10% happenes to man, the doctor told me that it won't cause any erection issues, and i don't have pain during or after intercourse.  The doctor said I have a "Classic Case of Interstitial cystitis" because when they filled my bladder full with water, traces of blood would appear.

I was given a drug called Elmiron, so I have been taking that.  Ever since than, my erection has been good.  However, for a month or so I stopped taking Elmiron becaus I ran out.  Than I didn't notice that general sympton such as pelvic pain and testicular discomfort getting any worse after I stopped taking the drug, but I noticed my erection is getting weaker again.

I am very worried about this situation, I am especially worried because I am 25 and I don't want to be dealing with erection problem for the rest of my life.  I am also curious as to why Elmiron is almost a little vigra for me, evertime I take it I get frequent/solid erection.  My questions are as follow:

1) Do IC cause erection problems?(I don't have pain during or after intercourse)

2) Could what I have be something other than IC? my sympton looks very much like PROSTATITIS.. but they would have checked that during the uroloscopy? Because I do know Prostatitis causes impotence.  If possible to be Prostatitis, what should I do?

3) Why does Elmiron allows me to get frequent erection? I really don't want to rely on drugs my whole life.

Thank you so much for your help....

To answer your questions:

1) I am not aware of IC leading to erectile dysfunction, although this may be possible.

2) I agree that prostatitis should be considered.  The cystoscopy would not properly evaluate this.  You can consider a prostatic massage and subsequent analysis of the prostatic fluid.  A transrectal ultrasound can be done to image the prostate for any abscesses.

3) I am not aware of Elmiron leading to a change in erectile quality.  Looking at the PDR, it does not mention an association with erectile function.

Followup with your personal physician is essential.
