Pain in epididymus head...strange symptoms

~9 months ago woke up with a sudden sharp pain on the epididymus of left testic. For next few months pain crept back, and I would move lft testicle around, or shake my leg, and I'd be all right. Just after summer of '06 Finally went to see urologist. He diagnosed it as Epididymitus (bsed on doppler-ultrasnd). Got tested for STDs, because Dr. thought it was cause. I'm still a virgin, and I don't do drugs, so I know it couldn't be STDs. Sure enough all tests came back negative (got tests second time; again tests were negative). He then said it was unspecified urithritis. Went on Suprax for 5 weeks, then Doxycyclene for 5 weeks (Dr. thought it was epididymo-orchitis or prostatitis), and then 4 days of Zithromax. Doxy helped a bit (one day I took two pills of doxy instead of one; the pain was unbearable; it helped very quickly, but just as quickly the pain returned). Zithro seemed to really bring things back to normal (but was only given it for 4 days; Dr. said too much was bad). Currently no burning pain. No Discharge. No problems urinating (except more frequently). If I hold my bowel movement pain gets worse (when I have my bowel movement pain is eased). If I don't urinate pain gets worse (pain is eased when I urinate). No problems ejaculating (I had to do that so I could get a semen culture). I'm tired, in pain, and miss being able to run. Sometimes when I lay down I feel this ease at the back of my scrotum, like some warm fluids are being released, and the pain disappears. Pain eventually returns. Please help me or point me in the right direction.

The ultrasound is a good initial test for the symptoms.  The workup and treatment clearly has been comprehensive.  

You can evaluate the prostate, which can refer pain to that area.  A digital rectal exam or transrectal ultrasound can help image the prostate.  Expressing prostatic fluid can be done via prostatic massage and sent for analysis and culture.  

You can also consider referred pain from non-infectious causes, like a urethral stricture or polyp.  A cystoscopy can be considered to evaluate for this.

These options can be discussed with your personal physician, or in conjunction with a urologist.
