I STRONGLY suggest having a MRI done to check for Tethered Spinal Cord. You can read up on all it's symptoms and make a check list. It needs to be a very sensitive MRI and if you can find a place that does ceniview MRI. I would ask that they include a full pelvis and your entire back. All 3 of my children had this and it took over 7 years for them to be diagnosed. CT Scans and Xrays won't show it and even some MRI's are not sensitive enough. Your description is almost identical to what my children experienced. GOOD LUCK!
