Please tell me of this has ever happened to anyone?

My son has been taking concerta for four years and has went up slightly with his weight gain over the years. He is 12 years old and is on 54 mg, and today he answered the door and then fell to the floor, started banging his head against the floor, and then got up walked over to the sofa and started watching tv. He had no recollection that he had even done that, and was completely conscious. Could this be due to the medicine?

Sounds like a mini seizure.  Meds may or may not have increased his chances of it (would be a bit weird after 4 years, but?).   And yes, I agree with Mark, you definitely need to talk with his doc about this.  He might want to adjust or change his meds.

Probably.  You should consult with the prescribing doctor about these behaviors.
